Outstanding Community Based Volunteer Award (CHB Individual Member) Award Criteria

This award is open to an individual Community Health Board (CHB) member who demonstrates the following:

• Engages community members with a focus on equity and social inclusion and reaching out to marginalized populations
• Evidence of building strong partnerships (collective impact) to develop upstream solutions to health
• Advocacy on the social determinants of health
• Innovative approaches to impacting community health and reducing barriers
• Demonstrated record of achievement or success
