Award Categories

Based on the award category, the customized set of criteria will prompt the nominator to provide a description of the nominee based on each criteria (that matches and/or exceeds the criteria for each award). If a group is being recognized, the nomination should list the group members and responses to award criteria will outline why and how the group worked together to achieve their goal(s).
 

Supplemental and Supporting Documentation (Optional)

Other supporting documentation such as photographs, news clippings, other awards of recognition, patient/provider testimonials, etc., may also be included in support of the nomination and may be uploaded as part of the nomination process.
