Oral Anticoagulant Management

Eligible: Patients requiring oral anticoagulants - Warfarin Sodium
Hours:  Monday, Tuesday and Thursday - 7:30am to 3:30pm
Location: Hematology Clinic 4South Victoria Building, VG site, Halifax, Nova Scotia
Staff: Dr. David Anderson, Medical Co Director & Steven Allen, Clinical Pharmacy Specialist Coordinator 

Referral Process - Regular Working Hours

  • Notify Anticoagulation Clinic at 902-473-6600 regarding your referral. An appointment will be booked for the patient to be seen in the Anticoagulation Clinic 4South Victoria Building, VG site on the next available Tuesday. Please notify patient with appointment.
  • Complete and fax Anticoagulation Clinic Referral Form.
  • Fax hospital interim discharge summary when complete. Include patient's medical problems, medications, recent warfarin doses, INRs, any bleeding history and a phone number with an answering machine.
  • Fill out the Blood Requisition for INR and CBC and give to the patient. Label top of requisition with the following information in Bold: Fax results to the Anticoagulation Clinic at 902-473-6812 and Please Return to Patient.
  • Ensure prescription for warfarin is written for 30 days, preferably for the 5mg strength.
  • Provide warfarin teaching. Contact Clinical Pharmacy Specialist covering your service.

Referral Process - After Hours

  • Leave a message on the Anticoagulation Clinic answering machine at 902-473-6600 regarding your referral. Include date of patient' s next INR, laboratory where INR will be completed and their warfarin dosage discharge instructions.
  • An appointment will be booked for the patient to be seen in the Anticoagulation Clinic on the next available Tuesday. The clinic will notify the patient with the appointment.
