Person Centered Ambulatory Care

The Person Centered Ambulatory Care team is a multi-service, multiple discipline ambulatory and community based team located throughout numerous sites within Capital Health. Our team is made up of occupational therapy, physiotherapy, psychology and social work clinicians.

The Cornerstones of Our Care are:

  • Chronic condition consultation / assessment / treatment / education
  • Collaborative interdisciplinary care to persons with chronic pain and diabetes
  • Multidisciplinary care within a number of community family practice clinics.

Our Ambulatory Care Services Include:

  • Outpatient physiotherapy services at the Nova Scotia Rehabilitation Centre
  • Pain Self-Management Program
  • Diabetes education
  • Psychology in the sleep disorders clinic
  • Social work support to the infectious diseases clinic
  • Psychology support within the obesity network
  • Physiotherapy and social work at Duffus Health Centre
  • Psychology at Dalhousie Medicine community clinics
