Outreach Program - Acquired Brain Injury

The Acquired Brain Injury (ABI) Outreach Team is a community based healthcare service for people who have an Acquired Brain Injury.

What does the ABI Outreach Team do?

We support people who have an Acquired Brain Injury, families and caregivers to live their best life.

The team will...

  • Build on the strengths and supports that a person with Acquired Brain Injury has so he/she can take part in activities and better manage their day. 
  • Support the person with Acquired Brain Injury with their move from hospital to home or other life changes that may come up.
  • Support the person with Acquired Brain Injury and other persons in his/her life to learn about the injury and issues that relate to it.
  • Work with the community to better meet the needs of the person with Acquired Brain Injury.
  • Help the person with Acquired Brain Injury solve problems that are unique to his/her needs and goals.
  • Support the person with Acquired Brain Injury’s families and caregivers in meeting his/her needs.

Who can be referred...

  • 16 years of age and older
  • Lives within a 25 km radius of the Nova Scotia Rehabilitation Centre, including Lower Sackville
  • Is ready and willing to work with the team to reach the goals agreed upon
  • Is open to including family or support persons in reaching goals
  • Has a documented acquired brain injury (Examples)
    • Traumatic brain injury - a blow to the head or hitting one’s head
    • Brain tumors
    • Brain infections (Example: meningitis or encephalitis)
    • Lack of oxygen due to heart attack or a near drowning event
    • Stroke

How to access our Service

We are currently accepting referrals from Capital Health healthcare providers only. To be seen by our service a completed Outreach & Day Program Referral Form form  is required. Fax referral to 902-425-6574 or mailed directly to our Coordinator.
