Completing the Laboratory Requisition

To prevent rejection or specimen testing delays, the requisition and specimen labels must be accurate, legible and complete; information on the requisition must match information on the specimen. The following links detail the requirements:

Special requirements for some testing are available in the Laboratory Test Catalogue.
(e.g.,  fasting required - no food or drink for at least 8 hours prior to collection)

Healthcare providers must provide this information and instructions to the patient prior to specimen collection. Guidance on medication use prior to collection is also required.
