Admission

Admission to Haliburton Place is done through the Single Entry process within Nova Scotia. The number to call is 1-800-225-7225 and the contact person will guide you through this process.

Respite Care

Respite Care is short-term care in Haliburton Place for individuals who are usually cared for at home by family members. Access to Respite Beds is coordinated through the Continuing Care Branch of the Department of Health. Application is made through the toll free number 1-800-225-7225. 

Social Work

The Social Work Department offers many services to Residents and their families. You may contact the Social Worker by making an appointment. The Social Worker works with the other members of the Team to ensure that the Residents’ physical, social, cultural and emotional needs are met.

Visiting Hours

Haliburton Place does not have any set visiting hours, as this is the Residents’ home. However, visiting hours for Hants Community Hospital are from 1:00pm to 8:00pm.  We ask visitors to Haliburton Place to only visit the Hospital Units during those times. Please do not visit if you are not feeling well.

Care Conference

Care Conference meetings are held at least once a year for each Resident in order for the Resident and/or family member to discuss the Resident’s Care Plan. A letter will be sent out to the Resident’s next of kin  one to two weeks before the meeting, stating the date and time. However, if you have any concerns at any time of the year, please feel free to speak with the Pam Christopherson, Unit Manager or the RN in charge.

Confidentiality

A Resident Waiver form must be signed to allow us to display a Resident’s name on their door and/or other locations within the Home. This waiver also gives Haliburton Place permission to have a Resident’s name appear on the Birthday List.

Resident Rights and Responsibilities

The Residents’ Bill of Rights and Responsibilities is displayed on the unit for easy accessibility. Each Resident is informed of this and shown where to find the display.

Concern & Complaint Process

Within the Capital Health District, there exists a formal process for concerns and complaints. Any day-to-day concerns that may arise can be directed to Pam Christopherson, Unit Manager who will investigate and report back to the Resident and/or family. Any major issues that arise must be dealt with in a formal manner requiring a documented case record.
