Joint Hypertension Initiative

The Diabetes Care Program of Nova Scotia [DCPNS], Cardiovascular Health Nova Scotia [CVHNS], and Renal Program of Care for the NSHA identified hypertension as a priority for provincial focus. With provincial attention on chronic disease prevention and management, these three programs and their many partners explored a variety of opportunities to positively impact hypertension in Nova Scotia. Efforts focus on increasing provider and public awareness of blood pressure, the approaches needed to prevent hypertension, the importance of early identification, and appropriate management.
 
An ongoing activity has been the “Come on Nova Scotia Blood Pressure Challenge.”  Launched in 2012, it has seen a continued focus during May (World Hypertension Month) on hypertension awareness activities.  Interested partners (Community/Primary Health Care providers and teams; community groups/organizations; businesses such as pharmacies, grocery stores, fitness centres, etc.; and workplaces request the Challenge Kits (including wallet cards, brochures, posters, promotional materials, tally sheet to track and report progress, etc.) and often conduct blood pressure screening events or deliver educational sessions, etc.
