How are Patients Referred?

All referrals must be sent electronically to from the referring clinician’s NS health email account. This is to ensure privacy and confidentiality of patients, as per PHIA and NSHA privacy office. Referral emails sent from an alternate email address are unable to be evaluated.
An online referral is available on the central zone intranet under "forms".
*If the referring clinician is unable to establish if the patient meets above criteria they should first evaluate the patient to determine appropriateness for the THFC service. 
E-mail referrals will be triaged according to urgency, this should be supported by a brief description in the referral email (i.e. decompensation / refractory symptoms, ED visits, frailty etc.). 
Urgent Referrals: If requesting an urgent referral (i.e. patient to be seen within 1 week) a phone consultation with the NP at 902-240-5267 and / or the physician on service via locating at 902-473-2220 is required.
