Upon completion of this session, participants will be able to: 

  • Explain the range of purposes for partnership building
  • Identify the levels of partnerships and relate them to their roles as primary health care providers
  • Identify existing and new potential partners, based on the determinants of health

Chronic Disease Self-Management Support 

Upon completion of this workshop, participants will be able to:

  • Understand key self-management and self-management support concepts and principles
  • Describe the provincially supported and available chronic disease self-management programs
  • Describe the “5 A’s” approach as a strategy to support patients with chronic disease self-management
  • Apply the three question interview as a technique for assessing patients’ readiness to change
  • Reflect on their current approach to working with patients living with chronic disease
  • Identify specific actions to improve chronic disease self-management support in their practice setting

Generations and Learning Styles at Work 

Upon completion of this session, participants will be able to: 

  • Explain self-awareness using the Johari Window Model 
  • Identify the sensory, social and cognitive components of learning styles 
  • Recognize the dominant characteristics of an individual’s learning style 
  • Describe strategies for adapting to different learning styles 
  • Identify the most critical generational influences for Baby Boomers, Gen Xers and Millennials 
  • Identify potential workplace motivators and irritants for Baby Boomers, Gen Xers and Millennials 
  • Use disclosure and invite feedback to increase self-awareness

Introduction to Cultural Competence

Session 1

  • Define population health as it relates to primary health care. 
  • Explain the linkages between population health, primary health care and primary care. 
  • Understand how population health and a focus on the determinants of health help to contribute to comprehensive primary health care. 
  • Reflect on their team’s approach to applying the social determinants of health in their primary health care setting. 
  • Understand health promotion as a principle of primary health care. 
  • Apply health promotion strategies and principles within primary health care services.
