Each year the Performance Excellence Program supports the Capital Health Quality Team Awards to encourage and reward our health care teams that have shown a true commitment to the improvement of quality health care. 

Applications

Quality improvement and CARE teams are encouraged to submit applications by the first Monday in December. To submit an application for the Quality Awards Competition your team will need:

• an executive summary
• a written report
• a poster

The top three award winning teams' projects will be submitted to the National Awards Competition sponsored by the Canadian College of Health Service Executives and 3M Health Care. The deadline for this annual national competition is February 1.
