Semen Analysis Specimens - Rejection Guidelines

The following specimens will be rejected:

• Delivered to the laboratory more than one hour after specimen collection
• Delivered to the laboratory outside of the operational hours, 8 a.m. to noon, Monday to Friday
• No requisition received with specimen
• Requisition has insufficient information (i.e. no time of collection)
• Specimen is collected in a condom
• Specimen is collected in an unsuitable container (non-sterile or not sealed)
• No name on the specimen container
• Partial name on specimen container
• No health card number on the specimen container
• Health card number on specimen container does not match health card number on Semen Analysis Request Form
• Patient name Semen Analysis Request Form does not correspond with health card number and date of birth
• No patient demographics on Semen Analysis Request Form
