Semen Analysis Specimens - Rejection Guidelines
The following specimens will be rejected:
- Delivered to the laboratory more than one hour after specimen collection
- Delivered to the laboratory outside of the operational hours, 8 a.m. to noon, Monday to Friday
- No requisition received with specimen
- Requisition has insufficient information (i.e. no time of collection)
- Specimen is collected in a condom
- Specimen is collected in an unsuitable container (non-sterile or not sealed)
- No name on the specimen container
- Partial name on specimen container
- No health card number on the specimen container
- Health card number on specimen container does not match health card number on Semen Analysis Request Form
- Patient name Semen Analysis Request Form does not correspond with health card number and date of birth
- No patient demographics on Semen Analysis Request Form
