Transitioning to Nova Scotia Health Authority from the IWK

Transition generally means a process of change from one stage to another. In the context of this initiative, transition refers to the purposeful, planned movement of youth with chronic conditions from paediatric to adult-based care. The IWK Health Centre and the Nova Scotia Health Authority (NSHA) support a planned and coordinated transfer of a patient and family receiving paediatric-based care to adult-based services. Through engagement and discussions with patients, families and staff we’ve developed a number of tools and processes to support this change.
