Mental Health Case Management Services

The Mental Health Program Case Management Services are examples of Best Practice in psychiatric rehabilitation. They provide a range of case management services based on two models, Assertive Community Treatment ( ACT ) and Intensive Case Management. The teams provide service to individuals living in the community with a severe and persistent mental illness or concurrent mental illness and substance abuse which has had a major impact on aspects of their lives, such as housing, employment, social supports, finances, daily living skills.

The teams operate under the Principles of Psychosocial Rehabilitation which emphasize hope, recovery, empowerment, and effective rehabilitation founded on a partnership between the person receiving services and the practitioner. The goal of the teams is to support individuals in achieving the best quality of life possible and to support individuals in their recovery and growth beyond illness.

The Supportive Community Outreach Team (SCOTeam):

  • Based on the Assertive Community Treatment Model
  • Multi disciplinary team approach
  • Provides Intensive Case Management Services
  • 7 days a week service
  • Individual assessment and flexible treatment and service planning
  • Supportive counselling
  • Direct assistance with medication and symptom management, medication monitoring and support, medical considerations
  • Assistance obtaining and meeting basic needs
  • Creating a supportive environment
  • Direct teaching / management of life skills and development
  • Assertive outreach and advocacy
  • Rapid and flexible responses to crises
  • Partnership-education and support of family members and significant others

Who We Serve

Adults who:

  • Have a serious and recurrent mental illness related to a psychosis or a major mood disorder
  • Have difficulty living independently in the community
  • Are at high risk of repeat episodes of acute illness
  • Have not benefited from traditional psychiatric outpatient or case management services
  • Have a history of frequent use of mental health and emergency services
  • Need flexible and intensive support to continue to live independently in their community.

How to Access our Services

Referrals:

  • Accepted from mental health professional in Capital Health
  • Contact Service Coordinator, Noel MacKinlay, at (902) 434-5027, Monday to Friday 8:30am to 4:30pm.

A case manager meets with the client and the referring clinician to obtain comprehensive clinical and daily functioning information. This is reviewed to determine which team can best meet the client’s needs. If the intake information indicates the client would be better served elsewhere, specific recommendations are made to the referring clinician.
