Quick Response Program

Capital Health’s Quick Response Program (QRP) is designed to provide home care services for 5 days or less to a person who arrives at the QEII or Dartmouth General Emergency Department with a medical diagnosis that has a predictable short term course of action, is unlikely to deteriorate, and manageable without extensive investigations or consultations. The Emergency Department’s Discharge Planner or Social Worker will set up the Quick Response Program and make a referral to Continuing Care to authorize service from the Red Cross and ongoing care management.

Home Again Program

Capital Health’s 60 day Home Again program provides individuals with time at home to consider the next steps in their care pathway including the possibility of admission to a Long Term Care facility.

To be eligible for the Home Again program, a person must have a home in the community to return to, be able to self-direct their care or have a Substitute Decision Maker who is capable and willing to direct their care and have an active support network around them.

Care by Design

Care by Design is transforming care for nursing home residents by providing them with on-site access to high quality primary health care services.

Hospital trips can be strenuous for nursing home residents. Often times, their medical needs can be addressed without visiting the hospital. Today residents have access to supports not previously available, reducing the need to visit the hospital.

Residents receive weekly regularly scheduled visits from a physician assigned to their floor, and through an on-call system, have access to urgent care 24 hours a day, seven days a week. They are supported by specially trained Paramedics who can assess and treat small injuries on-site.
