Capital Health Integrated Continuing Care connects you with options for care outside of the hospital such as, government home care and long term care programs. The goal is to help you live as safely and independently as possible at home and in the community.We connect you with one of our Continuing Care Coordinators, who will assess your needs, identify the programs and services that would best meet them, and set up the services.
