Doctors can refer individuals who have chronic lung diseases who remain symptomatic despite medical therapy.

  • Lung volume reduction or transplantation surgery
  • Pulmonary hypertension
  • Sleep apnea

The INSPIRED COPD Outreach Program

The INSPIRED COPD Outreach Program brings together a specialist team that is able to visit patients during a hospital admission and then later in their homes.

We Provide

  • Disease self-management education
  • Action plans for management of exacerbations of COPD
  • Care navigation
  • Psychosocial /spiritual support for “dyspnea crisis”
  • An opportunity for patients/family caregivers to engage in advance care planning (ACP) discussions
  • the opportunity to complete a Personal Directive if they choose

Eligibility

  • Community dwelling (i.e. not in nursing home or LTC)
  • Have a confirmed, pending or suspected diagnosis of moderate to advanced COPD
  • Living in the HRM Halifax Regional Municipality Central Zone (Halifax, Dartmouth, Sackville, East Hants, Eastern Shore), Cumberland County, or Cape Breton Regional Municipality
  • Willing to be referred

And Have at Least one of the Following

  • Admission to hospital with a flare up of COPD at least once in the past year If you live in Central Zone you may also be eligible if you have been admitted to the to the Intermediate Care Unit (IMCU) or the Intensive Care Unit (ICU) for a flare up of your COPD during the past year Gone to the Emergency Department more than once in the last year because of your COPD 
  • Admission to IMCU/ICU due to AECOPD in past year
  • Frequent use of ED services for AECOPD in the past year (with or without in-patient admission)
  • Moderate to Severe COPD
    • Too breathless to leave the house (MRC 5)
    • Breathlessness after dressing/undressing (MRC 5)
    • Stops for breath after 100 metres (MRC 4)
    • Walks slower than contemporaries or has to stop for breath when walking at own pace (MRC 3)
    • Chronic respiratory failure (PaCO2 > 45)
    • Clinical signs of right heart failure
